Atualização de checkbox, radio Button e menu

Estou fazendo este código em java script e não estou conseguindo deixar no estado inicial o checkbox (desmarcado), o radio Button(desmarcado), e a opção dos menu (selecione uma opção). Alguém poderia me ajudar?

Fiz a alteração, e continua não funcionando. Volta no estado inicial fechando a página e abrindo de novo.
